Incoming Wildcats
It is NMU Athletic Department policy that the following seven forms be returned before the student-athlete will be allowed to participate in any practice or competition..

• Student-Athlete Demographic
• Pre-preparticipation Evaluation
• Exercise-Induced Bronchospam
• Menstrual History (female athletes)
• Nutritional Supplement Disclosure
• NCAA Drug Testing Consent Form
  • Division I Drug Testing Consent Form (hockey)
  • Division II Drug Testing Consent Form (all other sports)
• Summer Drug Testing Contact Info
  
  Additionally, if you are medicated for ADHD with a stimulant, you need to read the following memo, per NCAA requirements, and provide the necessary documentation to the athletic training staff.

Should a student-athlete not provide the necessary documentation and if the student-athlete would fail a NCAA drug test because of stimulant use, the student-athlete would be suspended from their sport for one calendar year.

Complete the following forms as directed by your coach.  He or she will provide you with the deadline for when these need to be returned to the NMU Athletic Department.
