If you are unable to provide testing accommodations for a student due to lack of an available room or a scheduling conflict, please complete this form and deliver to Disability Services, 2001 Hedgcock.

Please allow 48 hours notice to allow for arrangements to be made.

Disability Services can provide test proctors, readers and scribes for students with approved accommodations.

All tests administered by Disability Services are proctored.

Test readers may read the test materials as often as the student wishes within the time allotted.

Test readers MAY NOT:

• re-word any test material
• define any test material or
• explain any test material.

Scribes write only what the student dictates. All punctuation, capitalization and grammar must be stipulated by the student.

Any irregularities that might occur during the testing period will be duly noted and returned with the test.
