Accommodations
Disability Services at Northern Michigan University provides assistance and accommodations to students who have documented disabilities. Disability Services works in cooperation with students, faculty and staff to ensure that students receive support for their disability needs.
Each accommodation request is reviewed on an individual basis. In order to receive accommodations the student is required to:
- schedule an appointment with the disability services coordinator at 906-227-1700;
- complete the Application for Disability Services, which can be picked up at the Dean of Students Office (2001 Hedgcock); and
- provide Disability Services with appropriate documentation of the disability (examples of appropriate documentation can be found below).
The following documentation is necessary to receive accommodations. Appropriate documentation is a written report submitted by a medical doctor, psychiatrist, psychologist, counselor or school psychologist.
Documentation
All documentation must be current (within the past three years).
For a cognitive, learning, psychological disability:
- diagnosis of a disability
- symptoms of the disability
- test scores and data which support the diagnosis
- recommendations regarding classroom accommodations
